Prospectus-cum-Application Form is available at the counter of the Admission Office, MM University, Mullana - Ambala (Haryana), 133-203 @1200/- (in the case of MBBS/BDS, MSc. Nursing & MDS) and Rs. 700/- (for all other courses) or select Branches of Axis Bank in Haryana, Punjab, Delhi, Chandigarh ,Himachal Pradesh, J & K, Uttaranchal, UP, MP, Bihar, Rajasthan, Jharkhand or from Budaniya Book Shop, Chandigarh and its branches on payment of Rs. 1250/- (in case of MBBS/BDS, MSc. Nursing & MDS) or RS. 750/- (for all other courses) on all working days.

The Application Form can also be downloaded from the University website. The downloaded filled in form should be accompanied by a DD of Rs. 1200/- (in the case of MBBS/BDS, MSc. Nursing & MDS) and Rs. 700/- (For all other courses) to avoid rejection. The candidates who have appeared for qualifying examination and expecting results can also appear for the entrance test.
