Any one of CAT 2011/XAT 2012/GMAT (taken or after August 2009) scores

1. Filling up an online application form available at IFMR website
2. Forms can be obtained from IFMR on payment of Rs. 1200/- by DD
3. Form can be also obtained from Bulls Eye/Career Launcher/IMS/T.I.M.E Coaching centers
